        Terms & Conditions

What We Need From You:

  1. A signed copy of the prescription,  including your Dentist license number, and the State in which the number is registered.  Please give us a telephone number and, if available, an email address for our records.

  2. A master model or a non-alginate impression. We DO NOT accept alginate impressions.

  3. Please wrap the models/impressions well. Send the case to us by US Mail or by a courier of your choice.

  4. We need a current Credit or Debit card on file. We accept Visa and MasterCard. Please contact Jodi at 303-665-7240 with the card info. We bill the case to your Credit or Debit card on the business day before the shipping day.

Case Design

If a Dentist-client gives us specific case design instructions and/or drawings, we will implement them. If there are no such details provided, we will use our default style for the appliance.

The default appliance styles we use accomplishes two goals:

We send you a case that works.

 Generally, the retainers of our default style are more comfortable to wear and can be easily adjusted as needed at time of delivery.

Our default unilateral space maintainer is one made with a single arm, soldered on the buccal surface of the band only. rather than the traditional continuous loop soldered to the buccal and lingual surfaces of the band. If the space we are to hold closes in the mouth before the appliance delivery, it is very  difficult to adjust a double arm continuous loop space maintainer to fit the smaller space. The single arm space maintainer (SASM) is easily adjusted to fit the smaller space at chairside, if necessary.

NB:

There is no difference in passive holding ability, comfort or effectiveness between the two designs of unilateral Space Maintainers noted above.

 We have fabricated and delivered over 3000 SASMs. Not one has been returned to us because the design was a failure in application.

• We limit the number of remakes.

The use of a horseshoe palate design in many of our maxillary Hawleys and retainers also reduces the failure rate and lead to greater patient comfort and compliance. If a patient doesn't wear a retainer regularly, in a very short time the appliance will not seat properly and we may see this ill-fitting retainer in our lab. 

If a space maintainer fails to seat, generally it is a problem caused by space closure. With one large lab client, we used to have a 9% failure rate, using the traditional double arm continuous loop style. That was very costly for both them and us. After some discussions about the problem, we rolled out the new "single arm space maintainer" (SASM) design. The failure rate is now below 2%, and some of that 2% are easily adjusted in the lab to fit a new model.

Satisfaction Guarantee

 We offer a full 30 day, no fault product guarantee, from the date of our invoice.

The original appliance must accompany a request for a free replacement appliance. If the original appliance is not sent, there will be a full charge for a new appliance. There must also be a new model or impression with the case to be remade. We will repair, revamp or remake the appliance, as we deem appropriate.

EXCLUSIONS:

I:  If the prescription for the appliance has changed, as to type of appliance, acrylic color, case design or, in the case of fixed appliances, the tooth or teeth to be banded, the Satisfaction Guarantee does not apply. People make mistakes, they can mix up quadrants and/or use the wrong tooth number or letter in a prescription. Neither you nor we are responsible for a Dentist-client error.

II:  If the integrity of the appliance has been compromised in any way, the Satisfaction Guarantee does not apply.

This includes, but is not limited to, any of the following:

Reduction of the height or thickness of an ortho band. Any such reduction compromises the strength and integrity of the band. Minor adjustments to the band may be covered, at our discretion, on a case by case basis.

Wires or clasps have ground on or reduced in thickness. This tends to lead to premature failure of the appliance.

III:   If the appliance has been modified, used in a manner for which it was not as originally prescribed or designed or reached maximum appliance opening or expansion, the Satisfaction Guarantee does not apply.

This includes, but is not limited to, any of the following:

A passive lingual arch is used in any manner to reposition a tooth or teeth. We offer a different design for an active lingual arch. The Dentist-client must initially request an active form of lingual arch or they will receive the passive form by default.

A passive Hawley is modified or added to, in order to effect tooth movement. If teeth need to be moved, tell us and we design the appliance appropriately and your Satisfaction Guarantee will be intact.

A fixed or removable expander or other device has been fully activated. If more activation is required, a full fee for a new appliance or revamp of an existing appliance, if we determine that this is possible, will be charged.

          Warranty

 We will warranty all your appliances that we fabricate for 6 months from date of our invoice against failure or breakage.

Case Turn Around Time

Our standard turn around time for a case is 5 working days in the lab. If we determine that a case will take longer, we will contact you immediately, so you can make the necessary arrangements.

Cases mailed to us should be sent by First Class mail. You can send them by Priority Mail if you are working in a narrow time frame.

We will return all cases to you by First Class mail.
